The subject was found dead with injection materials and several small plastic bags labelled with different DBZD 81. Clobromazolam was first identified in 20 white capsules containing white powder seized by Swedish Police in Luleå on 11 March 2016 11. There is limited information available on the pharmacology and toxicology of this substance, as there are no published reports so far. There are no clinically significant anecdotal reports from psychonaut fora describing the clinical effects of clobromazolam so far. Fonazepam was first identified in a sample of 51 tablets (27 white, 15 blue and 9 grey tablets) found in Lidköping, by Swedish police on 10 March 2016.

Flumazenil can treat patients experiencing a clonazolam overdose 70,71 who may present with somnolence, confusion, ataxia, hyporeflexia, bradypnea, and loss of consciousness 79,80,81. Metizolam is a thienodiazepine, structurally similar to etizolam, from which it differs by only a methyl ring in the thiophene moiety 117. Metizolam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs. Phenazepam has a half-life of up to 60 hours because it’s a long-acting benzodiazepine.7 The drug can potentially delay the onset of withdrawal symptoms for up to five days. Among the 6500 autopsy cases, the DBZs etizolam, flubromazolam, phenazepam, and diclazepam were detected in 21 cases.

  • The use of DBZs has been a subject of multiple recent clinical studies, likely related to increasing presence and availability on the internet drug market and lack of regulation.
  • The potent tranquilliser flualprazolam is being widely used in the four southernmost provinces of Thailand, officials say, after Regional Medical Sciences Centre 12 in Songkhla found traces of it in nimetazepam pills.
  • The benzodiazepines he used intermittently over the years had been obtained without prescriptions and included alprazolam and clonazolam, the latter categorized as a DBZ.
  • Furthermore, there were 28 additional deaths in which flualprazolam was not listed as the cause of death; these include 5 decedents from Finland, 13 from Sweden and 10 from the US.
  • In particular, a low dose is 0.5-1 mg, a typical dose is 1-2 mg, and a high dose is 2-4 mg or more 58.
  • There are no data about clinical, pharmacological or toxicological properties.

Chemical Structure

He was able to undergo successful buprenorphine induction for his heroin withdrawal management and benzodiazepine withdrawal management for three months. In the end, his cravings and worsening withdrawal symptoms resulted in a relapse of his DBZ use. According to the UNODC, the highest public health risk around the world is from etizolam, flualprazolam, flubromazolam and phenazepam 54,72. DBZD are widely available on the Internet in different forms, i.e., blotters, liquids, pills, powders and tablets, and sold at low prices 148. Etizolam and phenazepam are further diverted from the regulated market and illegally imported from those countries where they are licensed therapeutic drugs 138,149.

In 36, the cause of death was listed as accidental overdose due to multiple drug toxicity, while in 2 cases they were ruled intentional flualprazolam poisonings. Furthermore, there were 28 additional deaths in which flualprazolam was not listed as the cause of death; these include 5 decedents from Finland, 13 from Sweden and 10 from the US. Flualprazolam blood concentrations ranged from 3 ng/mL to 620 ng/mL 101,102. The substance was analytically confirmed by the Swedish National Forensic Centre using GC-MS 9, 135, There are no clinically significant anecdotal reports from psychonaut fora describing clinical effects of norfludiazepam so far. Bromazolam was first identified in a sample of 0.74 grams of yellow powder seized by Swedish Customs in Stockholm on 3 August 2016. The substance was analytically confirmed by the Swedish National Forensic Centre using GC-MS, GC-IRD (Gas Chromatography with infrared detection), LC-HRMS and NMR 9, 135.

Melatonin And Alcohol-related Disorders

An overdose can quickly lead to loss of bowel control, respiratory depression, coma and death. Deschloroetizolam is a short-acting thienotriazolodiazepine that differs from etizolam by the absence of a chlorine on the benzene ring with consequent reduced potency 1. On 1 September 2014, the UK Focal Point reported that the substance was confirmed after analysis of a blue seized tablet 50. In a self-administration study, one of the authors ingested one-half pink tablet of deschloroetizolam, about 6 mg, bought on the Internet 79. After 15 min, the subject’s overall behavior changed rapidly; both physical and cognitive effects were described. Deschloroetizolam and diclazepam’s metabolites, lorazepam and lormetazepam, were detected in a young male.

However, the number in 2016 was 225, with 300 in 2017 but an unprecedented 551 in 2018, out of a total of 1313 drug-related poisoning deaths 62. Etizolam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs. Diclazepam, or 2-Chlorodiazepam, is the 2’-chloro derivative of diazepam and the positional isomer of 4-chlorodiazepam 84. In two of three cases displayed, subjects were admitted to the ED in a severe state of agitation and disorientation; diclazepam was detected along with stimulants and dissociatives. Symptoms of intoxication were mainly characterized by CNS depression and a withdrawal syndrome. The patient reported having ingested two 30 mL vials of 4 mg/mL diclazepam (240 mg) purchased online.

However, banning drugs of a chemical class not yet discovered is legally very difficult. However, lots of drugs beat the “Analog” restriction for years, especially when the market is relatively small. That happens with some new anabolic steroids, routinely used by athletes or body builders to get their “edge.” They buy them online, like the effects, tell their friends — and a successful “product” begins its run. It’s occasionally used as a recreational drug in the U.S. for its euphoric effects.

Limitations Of The Present Study

It was found that the percentage of benzodiazepine users increases with age, and its use was nearly twice as prevalent in women compared with men 8. The highest prevalence of use was in 80-year-old women (11.9%) 8. The fraction of long-term prescriptions also increased from 14.7% in the 18 to 35-year-old category to 31.4% in the 65 to 80-year-old category 8. This presents a concern for benzodiazepine-related risks of falls, fractures, and motor vehicle collisions 17,18,19. In the analysis of benzodiazepine use in Canada, high and/or inappropriate prescribing of these drugs was highest in older adults 1.

How Long Do Its Effects Last?

Now and again, the length of treatment may be extended to 2 months. Like drinking and driving, driving while high is illegal – and you may still be unfit to drive the day after using phenazepam. You can get a heavy fine, be disqualified from driving or even go to prison.

7 Clonazolam

As with all benzodiazepines, clonazolam acts as a GABA receptor agonist, increasing the time that chloride channels are open 70. Clonazolam may also produce loss of motor control, amnesia, respiratory depression, dizziness, and muscle relaxation 87. The use of clonazolam has been found to antagonize the pharmacological effects of drugs like nicotine and potentiate the effects of central nervous system depressants such as alcohol 88. Flubromazolam (8-bromo-6-(2-fluorophenyl)-1-methyl-4H-1,2,4triazolo4,3-a1,4benzodiazepine) is a triazolo-analogue of flubromazepam 70. Despite its high potency and prolonged activity reported in recreational drug users, limited data support pharmacokinetic properties, physiologic effects, and detectability. A study of Flubromazolam‘s pharmacokinetics showed that the ingestion of 0.5 mg leads to multiple peaks in the serum concentration 71.

Again, 13 drivers apprehended for DUID submitted to a clinical test of impairment (CTI). The level of impairment was assessed based on the single test results and the individual’s general condition. Common signs of impairment were found for alertness, appearance, cognitive function, motor coordination and vestibular function. Subjects were aged between 30 and 39 years; sex was not specified, and diclazepam was found in blood at concentration ranging from 5.4 ng/mL to 32 ng/mL 86.

Phenazepam is used in the treatment of neurological disorders such as epilepsy, alcohol withdrawal and insomnia. It can be used as a premedication before surgery as it augments the effects of anesthetics and reduces anxiety. Alprazolam triazolobenzophenone has been firstly identified in a seizure of 1000 grams of white powder, seized at Madrid Airport in March 2014 by Customs authorities in a package that had arrived from India; it also contained paracetamol. The compound was identified and characterised using the gas chromatography-mass spectrometry (GC-MS) and nuclear magnetic resonance (NMR) by the Spanish National Focal Point 79. There are limited anecdotal reports from psychonaut fora but it is supposed to exert an effect similar to that of alprazolam 54. The Alprazolam triazolobenzophenone derivative represents a product of hydrolysis, under acidic conditions, of alprazolam and, hence, a metabolite of alprazolam as well.

In 2010, however, the USA (Bailey et al., 2010), and the UK in 2011 (Maskell, De Paoli, Seetohul, & Pounder, 2011), started to detect phenazepam, a drug that was at that time only available for prescribing in the former Soviet states, in drug-related deaths (DRD). These detections were the start of the rise of what are now termed new psychoactive substances (NPS) (also known as ‘legal highs’, ‘designer drugs’, and ‘bath salts’) throughout the world. The rise of the use of chemical compounds that have physiological and psychological effects but have not been properly tested in clinical trials like licenced medicines is worrying from both a social and medical angle with the drugs having unknown dangers.

Alcohol Rehab

Of sixty deaths reported, phenazepam alone was listed as the sole cause of death in two cases, while the remaining were attributed to accidental overdose due to polydrug toxicity. It was identified in Sweden in 10 seized white tablets labelled “XANAX” and reported to EMCDDA in October 2014 50. Eighteen patients were admitted to the ED in a severe state of CNS depression with functional and motor impairment. In 16 cases, flubromazolam was the sole drug detected, while in 2 cases subjects were also positive for meclonazepam. After logical verbal contact was established, he admitted that he bought flubromazolam on the Internet and consumed about 3 mg approximately 19 h before ED admission 106. Eleven flubromazolam DUID cases were reported; in two, driving impairment was assessed by CTI, while in the remaining nine, a SFST was performed by officers.

Other effects included anxiolysis, muscle-relaxing effects, euphoria, loss of control, and severe withdrawals 76. Prescribed benzodiazepines are considered among the safest medications, with low toxicity 13 and virtually no lethal potential if taken in isolation and as prescribed 34,36. However, they are certainly not benign, and misuse harms the patient. Determining the appropriate benzodiazepine to prescribe requires careful consideration of the illness being treated, the patient’s age, concurrent medications, duration of treatment, and the varying potency of the drugs 15,16. The most common side effects of benzodiazepines include dependence, rebound anxiety, memory impairment, and discontinuation syndrome 37,38.
